Outreach Team Connects Nevada Communities with Vital Services
From rural towns to busy city streets, MTM Health’s Nevada Community Outreach team has been hard at work making meaningful connections, removing barriers, and ensuring Nevadans know about—and can access—the services they need.
The team has participated in several recent impactful events, including the Nye County 10th Annual Social Affair, the Sunrise Children’s Foundation Head Start Programs Health Fair, a Caliente Community Resource Pop-Up, and Project Homeless Connect in Las Vegas.
At these events, Outreach Advocate Evelyn Miramontes and her colleagues shared information about MTM Health’s Nevada Medicaid non-emergency medical transportation (NEMT) benefit and Community Driver Program, helping residents understand how they can get to critical healthcare appointments or even volunteer to provide rides in their own communities. Many attendees also learned about the MTM Link Member mobile app, which makes it easier to submit mileage reimbursement claims.
These outreach efforts weren’t just about sharing resources—they were about making personal connections. Evelyn assisted members with app navigation, helped a mother access trip logs for her daughter, and connected families traveling out of state with mileage reimbursement options. She also worked alongside managed care organization partners to deliver consistent benefit information, ensuring families received clear guidance.
Some encounters were especially inspiring—like meeting Mike, a former Nevada Medicaid member who once struggled to get to medical appointments due to transportation barriers. After discovering our services at a community event, Mike was able to manage his health, secure employment, and find stable housing. Now, he’s paying it forward as a coordinator with the Nevada Homeless Alliance.
Whether in rural resource centers or large-scale health fairs, MTM Health’s Nevada Community Outreach team continues to make a tangible difference—one conversation, one ride, and one connection at a time.
