MTM Hires Jim Sebben in Home and Community Based Services Director Role

MTM, Inc. announced today that it has hired Jim Sebben as its Director of Home and Community Based Services (HCBS), MTM’s newest service offering. Driving a paradigm shift in healthcare, MTM has adapted its successful transportation management model to redefine personalized care coordination and transform the delivery of quality home care to the increasing aging and dual-eligible populations.

In his role, Jim will oversee the development and operations of coordinating home and community based services for individuals who are seeking to stay in their homes. Through home and community based services, MTM’s goal  is to help managed care clients improve health outcomes, reduce fragmentation, provide enhanced person-centered care, and realize significant cost savings.

Jim’s background includes work as the Director of Home Care for St. Louis Jewish Hospital; Senior Product Developer for Blue Cross Blue Shield of Missouri; Program Director of a physician network, pharmacy, and outpatient services for long-term care facilities for Deaconess Health Systems; and Vice President of Business Development with MTM for six years.

Jim holds a Doctorate of Philosophy degree with a concentration in Social Science Research from St. Louis University; a Masters of Arts with a concentration in Social Work from St. Louis University; and a Bachelor of Arts in Philosophy from Creighton University.MTM is a medical and transportation management company that removes community barriers for five million people annually in 28 states and the District of Columbia. MTM manages transportation, care coordination, call center activities, ambulance claims, and functional assessments and travel training for state and county governments, Medicaid and Medicare managed care organizations, and healthcare providers. These entities rely on MTM to meet quality and safety standards while containing costs.
