MTM is always recruiting high quality home and community based service providers to become part of our healthcare network. In particular, we seek the following providers for networks throughout the country:

  • Home health aides and skilled nurses
  • Homemaking and Personal Care Assistants (PCAs)
  • Home infusion and pharmacy providers
  • Durable Medical Equipment (DME) providers
  • Home modification contractors
  • Meal providers
  • Therapy and rehabilitation assistants
  • Translators
  • Pest control solutions
  • Equine, canine, and art therapists
  • Transportation providers

Joining the MTM network consists of application, credentialing, and contracting phases. You will work with a designated Network Development Representative to help you through each phase.

Letter of Intent

The provider completes and submits a Letter of Intent, which details the types of services provided, service area, and all company contact information.

Credentialing

The provider submits insurance documentation, employee information, and completed criminal background checks (where applicable). MTM’s Network Development Representative conducts an initial site visit to evaluate the agency’s business practices, IT capabilities, and equipment. While there, the Representative introduces the provider to the MTM Provider Guidelines. The results of this phase determine whether the provider moves to the contracting phase.

Contracting

MTM drafts a Service Agreement/contract and reimbursement schedule between MTM and the provider, which includes the extensive requirements of the MTM Provider Guidelines. Once the provider signs the contract and MTM countersigns, the provider officially becomes a member of the MTM home and community based services network and is eligible to begin receiving work.
