MTM designed the transportation management industry’s first Utilization/Care Management (UCM) Department. This department gives medical facilities, social workers and caseworkers a single point of contact when scheduling non-emergency transportation. We staff this department with specially trained Care Managers who receive oversight from a Registered Nurse. Together, their role improves coordination and customer satisfaction with the transportation program.
Care Managers-
coordinate special trips such as dialysis, chemotherapy, high-risk obstetrical care, behavioral health services, transplants and hospital discharges.
establish solid relationships with social workers, case managers, facility staff and hospitals, providing a direct access point for one-on-one communication
Conduct level-of-need assessments
Arrange commercial bus, rail and air transportation
Understanding how to use the transportation benefit is necessary for a successful program. Our UCM department conducts education, training and outreach (ETO) sessions with local medical facilities and advocacy groups in our service areas. The sessions provide these stakeholders with clear understanding of how to access transportation, pertinent contact information, and addresses specific questions unique to the setting. The education, training and outreach efforts reduce concerns often associated with change and increases satisfaction with the facility staff and other stakeholders using the program.
ETO sessions continue after start-up and as needed to ensure a smooth transition to the NET/NEMT program and good customer satisfaction.
“Our transportation services are very limited and often I must find services on an immediate basis for our dialysis patients. MTM has been very responsive in these kinds of situations”
