Client Resources

MTM will help set up rides for people in the Texas Medicaid and Children with Special Health Care Needs programs who do not have another way to get to covered healthcare appointments with their healthcare providers.

To find out if you can get a ride, call MTM at 1-855-687-4786.

MTM uses your doctor’s feedback to determine the type of ride you need. Your doctor will fill out a Healthcare Provider Statement of Need form. Click here to see how we decide what type of ride to offer, including:

  • To pay you back by the mile if you drive
  • Mass transit tickets
  • A sedan, van, or taxi ride
  • A car or van that can carry a wheelchair, if you need that kind of service
  • Advance funds for meals and lodging
Children 15 to 17 years old can ride without a parent, guardian, or attendant when MTM has a signed Parent Consent Form for those teens. Children 14 years old or younger must ride with a parent, guardian, or attendant at all times. If the attendant for a child 14 or under is not a parent or guardian, a Parental Accompaniment Form must be on file. Teens who have chosen to live without parents or a guardian (emancipated teens) can ride without an adult.
Call the “Where’s My Ride?” phone number at 1-888-513-0706 and ask that someone come pick you up. We’ll send someone to pick you up in less than an hour.
If you disagree with a denial or other decision made by MTM you have the right to appeal. You can do this by calling or writing to us within 90 days of the date on your denial letter. You must tell us you disagree with our decision. Send your letter to:
