Joining the MTM network consists of application, credentialing and contracting phases.
You will work with a designated Network Representative and an Area Liaison to help you through each phase.
Application – The transportation provider completes and submits a questionnaire, which details the types and number of vehicles, service area, hours of availability, levels of service provided, and all company contact information. Click here to download a Transportation Provider questionnaire.
Credentialing – The transportation provider submits insurance documentation, driver lists, and completed driver criminal background checks (where applicable). MTM’s Area Liaison conducts an initial site visit to evaluate the adequacy of the transportation provider’s office location and equipment. While there, the Liaison also performs vehicle inspections and introduces the provider to the MTM Transportation Provider Guidelines. The results of this phase determine whether the transportation provider moves to the contracting phase.
Contracting – MTM drafts a Service Agreement/contract and reimbursement schedule between MTM and the transportation provider, which includes the extensive requirements of the MTM Transportation Provider Guidelines. Once the transportation provider signs the contract and MTM countersigns, the transportation provider officially becomes a member of the MTM transportation network and is eligible to begin receiving trips.
The Area Liaison and Network Representative will conduct ongoing transportation provider education and support as needed.
