MTM has created special partnerships with facilities to assist in simplifying the arrangement of transportation for their patients. We make it easy from the passenger’s viewpoint as well as from the medical and service professional’s perspective. We know the importance of communication, education, and coordination to running a successful program.
Communication: Specially trained Care Managers coordinate transportation services for dialysis patients, drug and alcohol programs, day care programs, psych/social programs, nursing home residents, hospital discharges, gas, food, lodging and out-of-state or exceptional travel depending on our Client’s protocols. In some instances, illnesses, or treatment programs, MTM can arrange trips for up to six months at a time. This streamlines the process for all parties-- the facilities, the transportation provider, and the passenger.
Education: Prior to implementing a transportation program, MTM staff will meet with facility staff for education, training and outreach (ETO), which includes new program orientation. Following implementation of a transportation program, our staff will provide additional in-service training as needed or requested. Our goal is to relieve facility staff of the time–consuming task of arranging appropriate transportation for their clients. Through our processes, we not only save our clients time, we stretch their transportation budgets.
Coordination: In some markets, MTM coordinates the distribution of public transit tokens or passes directly to facilities for distribution to their clients. This effort helps facilities improve program attendance, treatment compliance, and serves to reduce inappropriate use of the passes or tokens. Reducing waste stretches the transportation budgets of our clients.
